To effectively help you, we must receive your last 3 years of medical records, in addition to all completed intake forms at least 72 hours before your appointment. Please note: if we do not receive at least 48 hours advance notice for New Patient appointments and 24 hours advance notice for established patients, you will be charged for the appointment slot you have held. 

All payments are due at time of scheduling, in cash. We accept major credit cards, cashier's checks, and bank fund transfers via Paypal. We do not accept personal checks.
